Insurance
Primary insurance claims on your account will be filed
automatically. Additional claims will be filed at your request.
Please call our Insurance Department to discuss any questions
you have regarding insurance claims.
It is extremely important for us to have correct information
so that we can file your insurance claim accurately. We ask
every patient to present their insurance card at each visit.
Credit Policy
You may be requested to pay for your medical services
on the date the services are provided. The office visit copays
due from patients on managed care plans should be paid
prior to the appointment.
Insurance claims will be processed automatically. However,
it is your responsibility to pay the balance of your bill promptly.
Payment in full is expected unless a payment schedule has
been approved by our Patient Accounts Department.
A finance charge will be imposed on balances that reach
90 days past due. A 1.5% finance charge (18% annual rate)
will be charged on the outstanding balance.
The clinic accepts Visa, Mastercard and Discover in order
to make payment more convenient for you.

any questions regarding our credit policy.
Fees and Billing
Fees are based on the nature of services you receive. Fees
for your initial visit may be higher because of the additional
time necessary to establish your medical history.
Ask the receptionist at the appointment desk in your
physician’s area for a statement of charges if you wish to pay
when you leave. Otherwise, you will receive an itemized
statement in the mail, including the date that your claim was
filed with your insurance carrier.

Medicare and Medicare Supplements
Your Medicare claim and Medicare supplement are filed
automatically for you.
Medical Assistance
Medical Assistance claims are filed automatically.
When you register for your appointment, you must
present your current Medical Assistance card to the
receptionist. We reserve the right to refuse treatment for
visits other than emergencies unless you have your current
insurance card with you.
